Whipple procedure explained

The Whipple procedure (pancreaticoduodenectomy) removes the head of the pancreas, the duodenum, the gallbladder, and a portion of the bile duct. The remaining pancreas, bile duct, and stomach are then reconnected to the small intestine. It is a 6 to 10 hour operation requiring a high level of surgical expertise.

Who needs this?

  • Cancer of the pancreatic head, ampulla of Vater, or lower bile duct
  • Some neuroendocrine tumors

Many patients are told their pancreatic tumor is inoperable based on initial imaging. A second opinion from an HPB surgeon often reveals that surgery is in fact possible, especially with modern neoadjuvant chemotherapy.

Risks

Pancreatic surgery has higher complication rates than other GI surgeries. Risks include pancreatic leak, delayed gastric emptying, bleeding, and infection. Outcomes are best at high volume centres with experienced HPB surgeons.
